Bulimia nervosa is a diagnosable psychiatric condition with a strong evidence base for treatment. Cognitive behavioral therapy is the most well-studied and effective treatment, and medication has a clear, specific role alongside it. My part is the psychiatric side: evaluation, medication when it's appropriate, and coordination with your therapist and primary care physician so the physical and psychological sides of treatment work together.
The initial evaluation covers eating patterns, physical symptoms, and the depression or anxiety that frequently occur alongside bulimia. I also coordinate with your primary care physician on the medical monitoring bulimia can require, since it affects electrolytes and cardiac health in ways that need to be tracked, not just the psychiatric symptoms.
Fluoxetine is the only medication with an FDA indication specifically for bulimia nervosa. It's typically used at a higher dose than for depression, around 60mg daily, and clinical trials have shown meaningful reductions in binge and purge frequency at that dose. I explain the dosing rationale clearly so it doesn't feel arbitrary.
The best outcomes come from combining fluoxetine with CBT rather than using either alone, particularly when CBT hasn't produced a substantial improvement after about ten sessions. If you don't already have a therapist experienced in eating disorders, I can help connect you with one. With your consent, I coordinate directly with your therapist.
Not every antidepressant is a safe choice for bulimia. Bupropion in particular carries a meaningfully elevated seizure risk in patients with bulimia and is generally avoided. Part of a careful medication plan is knowing what not to prescribe, not just what to prescribe.
Follow-up appointments track binge and purge frequency, medication response and side effects, and relevant physical symptoms. Relapse is common even with treatment, so ongoing care matters past the point symptoms first improve. Care is available by telehealth across New Jersey and New York once established.

Warning signs of bulimia include eating large amounts of food in secret, going to the bathroom right after meals, frequent dieting or fasting, excessive exercise, swollen cheeks or jaw, calluses on the knuckles, and dental damage. Weight is often in the normal range, so bulimia can go unnoticed for years.
This isn’t unusual. Relapse is common even with treatment, and if CBT hasn’t produced substantial improvement after about ten sessions, adding or adjusting medication is a standard next step. I look at your full treatment history to figure out what hasn’t been tried yet.
