Another Perspective on Your Diagnosis and Medications

Asking for a second opinion doesn't mean your current provider got something wrong, and it doesn't require their permission. It means you want an independent read on your diagnosis and medication plan before making a decision, whether that's because something still isn't working, your provider is retiring or unavailable, or you're helping a family member make sense of a regimen that's grown complicated. This is a single, focused consultation built for exactly that.

WHAT I REVIEW

Bring whatever you have: a current medication list with doses and how long you've been on each, prior diagnoses, records if you have them, and a description of what has and hasn't helped. If you don't have formal records, that's fine too, we work from your account of your history instead. I go through the full picture before the appointment so the time we spend together is spent on judgment, not paperwork.


WHERE SECOND OPINIONS USUALLY FIND SOMETHING

A few patterns show up often enough to be worth naming. A diagnosis assigned during a brief or crisis visit that was never revisited as more history became clear. A medication called ineffective after a trial that was too short or a dose that was never actually reached. A second condition, commonly ADHD, an anxiety disorder, or a sleep problem, that was never screened for and is quietly working against whatever else is being treated. None of this means anything was done wrong. It means the picture had more angles than a rushed first look could capture.


WHEN THIS IS PARTICULARLY WORTH DOING

A second opinion is most useful when you're on two or more medications and unsure whether each one is still earning its place, when you're considering a major change like stopping a long-term medication or starting a controlled substance, when your diagnosis was made quickly and never revisited, or when your provider is retiring, relocating, or otherwise no longer available and you need continuity of judgment before choosing what comes next.


NON-STIMULANT MEDICATIONS

When stimulants are not appropriate or well tolerated, non-stimulant options such as atomoxetine (Strattera) or guanfacine may be considered. These can suit adults with certain cardiovascular concerns, a history of substance use, or co-occurring anxiety. They generally take longer to reach full effect and are titrated gradually, with monitoring for side effects, as part of an individualized plan.


WHAT YOU LEAVE WITH

After the appointment, you receive a written summary: my read on your diagnosis, specific thoughts on where your current medications could be optimized, and next-step recommendations. You can bring that back to your existing provider, use it to decide whether to continue treatment as-is, or use it as the basis for starting ongoing care here. Which of those you choose afterward is entirely up to you, this consultation doesn't commit you to anything beyond itself.

Why molecular biology matters to a second opinion

Before clinical practice I spent over ten years in molecular and cellular biology research at the Weizmann Institute of Science and Mount Sinai. That training is about isolating variables and not accepting an unverified result, which is exactly the discipline a second opinion needs: not a fresh guess, but a systematic check of what was tried, at what dose, for how long, and whether it was ever given a fair test.

Things patients usually want to know before reaching out.

Do I need my current provider's permission or a referral?

No. A second opinion is a normal part of psychiatric care, and most providers regard it as routine, especially for complex cases. You’re entitled to one regardless of whether your current provider is involved or even aware.

What if I don't have my medical records?

The appointment still works. Bring whatever you have, even if it’s just your own account of what you’ve been prescribed and how you’ve responded. Having a formal record makes the review more precise, but it isn’t a requirement.

Can I get a second opinion for a family member, like a parent seeking a review for their child or an adult child helping a parent?

Yes, this is common. Bring what medication history and records you have access to, and we’ll work from there. If the person themselves will be part of the appointment, that helps, but it isn’t always necessary depending on the situation.

My psychiatrist is retiring or no longer available. Is this the right service?

Yes, this is one of the most common reasons people request a second opinion here. Rather than starting from zero with a new provider, this consultation gives you an independent read on your current regimen and a written summary, whether you use that to transition into ongoing care here or take it elsewhere.

Will you tell my current provider what you found?

Only if you want that. The written summary is yours. You can share it with your current provider, keep it for your own reference, or use it as the basis for switching care entirely.

Can adults be diagnosed with ADHD if they never had it as children?

ADHD usually begins in childhood, but symptoms are often overlooked or attributed to other causes, so many adults are diagnosed for the first time later in life. Inattention, disorganization, and difficulty with follow-through may only become disruptive under adult demands. With an accurate diagnosis, treatment can combine medication, therapy, and targeted lifestyle changes for meaningful improvement.

Does this commit me to becoming a patient here?

No. This is a single, bounded consultation. Some people use it as the start of ongoing care here, and many use it exactly as intended: one independent opinion, then they decide what to do with it on their own.

Why isn't this covered by insurance the way other visits sometimes are?

Second opinion consultations are rarely covered even by plans that reimburse other out-of-network psychiatric visits, since many insurers treat them differently from ongoing care. I still provide a superbill in case your plan covers it, but I’d encourage checking with your insurer directly before assuming either way.

How long does the appointment take, and is it available by telehealth?

Up to 50 minutes, and yes, available by telehealth anywhere in New Jersey or New York, or in person at the Fort Lee office.
