ADHD Medication Without Insurance: Real Costs in NJ and NY

Paying for ADHD medication without insurance means covering two costs: the prescriber visits and the medication itself. As of November 2026, generic stimulants such as amphetamine salts XR and methylphenidate ER can cost roughly $15 to $45 a month with a discount card, against average retail prices of $200 or more. At Gimel Health, the evaluation is $350 and each follow-up is $300.

Most cost guides cover only the pharmacy price. That leaves out the part that adds up over a year: stimulant prescriptions can’t be refilled, so ongoing visits are part of the cost of treatment. This guide covers both, with prices dated so you know how fresh they are. If you’re in New Jersey or New York, Gimel Health offers ADHD medication management on a self-pay basis with Michael Feldman, PA-C.

What You Pay for ADHD Medication Without Insurance

You pay for two things, and they behave differently. The medication is a recurring monthly cost that varies a lot by drug, strength and pharmacy. The visits cost the most in the first few months, while the dose is being adjusted, and less once things are stable.

Generic or brand matters more than anything else on the pharmacy side. Generic stimulants with a discount card are often a fraction of their retail price. A brand-only drug with no generic version can cost several hundred dollars a month whatever you do. Which medication fits you clinically comes first, but if two options are equally suitable, cost is a fair thing to raise with your prescriber.

Generic vs Brand Prices (Checked November 2026)

These are cash prices for 30 days’ supply from GoodRx, comparing the lowest coupon price listed with the average retail price. Prices vary by pharmacy and ZIP code and change often, so treat them as a guide and check your own pharmacy before you fill.

  • Generic Adderall XR (amphetamine salts XR): from about $15 to $28 with a coupon, against an average retail price of $207.68 for the most common version (GoodRx, checked 5 November 2026). Two GoodRx pages for the same generic showed different “as low as” figures on the same day, which shows how much prices move.
  • Generic Vyvanse (lisdexamfetamine): from $62.28 with a coupon, against $301.91 average retail. Brand Vyvanse chewable tablets were about $97 to $99 for 30 tablets with a coupon (GoodRx, checked 5 November 2026).
  • Generic Concerta (methylphenidate ER), 30 tablets: about $39 to $42 with a coupon at 18, 27, 36 and 54 mg, against $349 to $394 retail. The 45 mg and 63 mg strengths were around $311 even with a coupon (GoodRx, August 2026).
  • Generic Strattera (atomoxetine) 40 mg, 30 capsules: $15.30 with a coupon, against $195.41 retail. That figure is from GoodRx in September 2024, so it’s older than the others.
  • Qelbree (viloxazine) 200 mg, 30 capsules: $398.67 with a coupon, against $483.79 average cash price. There’s no generic version (GoodRx, May 2025).

The methylphenidate ER figures carry the most useful detail. Strengths that cost more than the others can sometimes be replaced with a combination of cheaper strengths that adds up to the same daily dose. That’s a question for your prescriber and pharmacist, not something to change yourself.

Availability matters too. In 2024, a CDC study in MMWR (ADHD diagnosis, treatment, and telehealth use in adults) found that 71.5% of adults taking stimulants had trouble filling a prescription because the medication was unavailable. The cheapest pharmacy is no use if it doesn’t have your strength, so calling ahead saves both money and time.

Visit Fees at Gimel Health

At Gimel Health, the initial evaluation is $350 for up to 50 minutes, and each follow-up is $300 for up to 25 minutes. A free discovery call of about 10 minutes comes first, so you can check it’s the right fit before paying anything.

The number of follow-ups is what changes your total. Here’s an illustration, not a quote: if your first three months included the evaluation and two follow-ups, the visits would come to $950 before medication. How many visits you actually need depends on how quickly your dose settles and which medication you’re on.

How Often Do You Need a Visit for Refills?

Stimulants can’t be refilled under federal law, so each new supply needs a new prescription. Under 21 CFR 1306.12, a prescriber can issue several dated prescriptions covering up to 90 days where state law allows it. New York generally limits each controlled-substance prescription to a 30-day supply.

In practice, visits are most frequent while the dose is being adjusted and spread out once it’s stable. Non-stimulants such as atomoxetine and viloxazine aren’t controlled substances, so their refills work like any routine prescription. That can mean fewer visits purely for prescription purposes. The guide to stimulants vs non-stimulants explains the clinical differences behind that choice.

How to Lower the Cost

A few habits cut the monthly total without touching your treatment:

  • Ask for the generic. Every stimulant listed above except Qelbree has a generic version, and the gap between generic and brand is often hundreds of dollars a month.
  • Compare pharmacies. The same prescription can vary widely in price between stores a few miles apart. GoodRx and similar tools let you compare before you fill.
  • Check the strength. As the methylphenidate ER prices show, one strength can cost several times more than its neighbours. Ask whether the same dose can be made up from cheaper strengths.
  • Call ahead. Confirm the pharmacy has your exact strength in stock before the prescription is sent, so you aren’t forced to pay more elsewhere at the last minute.
  • Ask about manufacturer programs for brand-only drugs. For Qelbree, GoodRx reported in May 2025 a manufacturer discount for cash-paying patients of up to $150 off 30 capsules. Check the current terms, because they change.
  • Space out visits once you’re stable, in agreement with your prescriber, rather than skipping them.

Don’t stretch your supply by skipping doses. If cost or a shortage is about to leave you without medication, tell your prescriber, who can look for alternatives.

Getting Money Back From Insurance

If you have insurance but see an out-of-network prescriber, you may be able to claim part of the visit fees back. Gimel Health provides a superbill, an itemized receipt you submit to your insurer, for every visit. How much comes back depends entirely on your plan’s out-of-network mental health benefit and your deductible. The superbill guide walks through submitting a claim step by step.

Medication works differently. A discount card is used instead of insurance, not alongside it, and purchases made with one often don’t count toward your deductible. If you have pharmacy coverage, compare your copay with the discount price before each fill. The cheaper option can change from month to month.

Frequently Asked Questions

Usually, by a wide margin. In November 2026, GoodRx listed generic Adderall XR from about $15 to $28 a month with a coupon, against an average retail price of about $208. Prices vary by pharmacy and strength, so compare a few nearby pharmacies and confirm they have your strength in stock before the prescription is sent.

Stimulant prescriptions can’t be refilled under federal law, so each new supply needs a new prescription. Federal rules allow up to 90 days of dated prescriptions where state law permits, while New York generally limits each prescription to 30 days. Visits are most frequent while your dose is being adjusted.

Usually, yes. Discount cards like GoodRx are used instead of insurance, not with it, and the price varies by pharmacy. GoodRx notes that some states restrict coupons on certain controlled substances, so ask your pharmacist. Discount-card purchases often don’t count toward your insurance deductible.

It depends on the drug. Generic atomoxetine has been as low as about $15 a month with a coupon. Qelbree has no generic version and cost about $400 for 30 capsules with a coupon in 2025. Non-stimulants also aren’t controlled substances, which can mean fewer visits just for prescriptions.

Possibly. Gimel Health provides a superbill for every visit, which you can submit to your insurer for out-of-network reimbursement. How much comes back depends entirely on your plan’s out-of-network mental health benefit and your deductible, so check with your insurer before you book.
